Community Care – Home Based Services

The Home and Community Care (HACC) Program provides a range of services that can support people living with a disability to remain independent and living in the community.  When people stay active, involved and doing as much for themselves as possible, it is proven to help people stay independent with a greater sense of wellbeing.

Eligibility for the HACC program is assessed prior to commencing any service, and is targeted for people with disabilities and their carers.

Services Provided
Home Care:
Our Community Care Support Workers work with you to keep your house clean and safe, and assist with undertaking shopping and bill paying.  This service is available Monday to Friday (except for public holidays).

Personal Care Provides assistance with showering, toileting, bathing and dressing and other non-nursing tasks.  This service is available 7 days a week, including public holidays.

Property Maintenance
Property Maintenance provide a service for minor repairs to the home to reduce safety concerns and modifications necessary to maintain the security and safety.  Structural works or tasks requiring qualified tradespeople cannot be undertaken.

Examples of service include:

  • Installing ramps and rails
  • Installation of smoke detectors and replacement of batteries
  • Replacement of light globes
  • Seasonal cleaning of spouting
  • Installation of handheld showers

Property Maintenance is not an emergency service and is only available Monday to Friday.

Eating Well
There are a number of options when it comes to maintain nutrition and eating well.  Our Community Care Support Workers can work with you to undertake shopping and meal preparation within your own home.

We can also arrange to have pre-prepared meals delivered to your home (Meals on Wheels).

Living At Home Assessments Our qualified Living At Home Assessment staff meet with people in their own home to identify their strengths and explore a range of ways to remain living independently and remain active members of their community.
